I understand that owning a companion animal has legal, moral and financial responsibilities, and in making this application I affirm that I am willing and able to accept these responsibilities. I am aware that my animal must be under my control at all times, and I accept legal and financial responsibility for any damages my pet may cause in the future. I agree to comply with all Animal Control ordinances in my area and agree that I will not allow the animal to become a nuisance to the community, or endanger its own safety, by running at large. Further, I agree to treat the animal in a humane manner, providing it with wholesome food, proper and clean shelter, and regular veterinary care including yearly vaccination. I understand it is a violation of this agreement to transfer this animal to any other party without the written consent of Save A Lab. I further agree that should I violate any portion of this agreement, that legal action may be taken against me. I agree that while Save A Lab seeks to adopt me an animal who is in good health, they do not guarantee it to be free of congenital or breed-disposed diseases (hip dysplasia, hypothyroidism, etc.). Therefore, I relieve Save A Lab of any responsibility for the health of this animal now and in the future, and for any expenses I may incur. I agree to return the pet to Save A Lab if I am unable to keep it for any reason. I understand that my adoption fee is non-refundable.
